Memorandum: Important Information Regarding Workers' Compensation Benefits

Effective October 1, 2007, Self-Insured School of California (SISC-I) implemented a Medical Provider Network (MPN).  The purpose of the MPN is to ensure that every employee who has a work-related injury obtains medical care quickly, is treated appropriately, and safely returns to work as soon as medically possible.  Please Note: Unless exempted by law or by the employer, all medical care for workers injured on the job whose employer has an approved MPN will be handled and provided through the MPN.

If you choose to pre-designate your personal care physician in writing prior to being injured at work, the pre-designation form is available on-line under HR Forms @ http://www.imperial.edu/Media/33-DWCForm_9783-(pre-designation).pdf.

Also available online is a Medical Provider Handbook, which describes in detail the Self-Insured Schools of California Medical Provider Network (MPN).  The document outlines how to access the MPN, how to find a physician, how to obtain medical treatment, changing providers, and other information related to the network.  We ask that you read this information carefully.
